No Surprises Act Notice

Your Right to a Good Faith Estimate

Important Notice for Uninsured or Self-Pay Patients

Effective January 1, 2022, the No Surprises Act provides new protections against surprise medical bills for patients. As a self-pay/cash-pay practice, we want you to understand your rights.

Your Right to a Good Faith Estimate

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, healthcare providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
  • You can ask your healthcare provider for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

How We Comply

Bravely Balanced is committed to price transparency. As a cash-pay practice:

  • All of our service prices are clearly displayed on our website
  • We provide written estimates before any service is performed
  • We will inform you of any additional costs before they are incurred
  • You will receive a detailed receipt after each service

Disputing a Bill

If you are billed for more than $400 over your Good Faith Estimate, you have the right to dispute the bill. You may contact the healthcare provider to inform them of the error and request an adjustment. If you are unable to resolve the issue directly with us, you can start a dispute resolution process with the U.S. Department of Health and Human Services (HHS).
